Required forms

The YOUTH CODE OF BEHAVIOR must be completed by every student 12 years and older. One form per child. Email the form to Ms. Riley Oliver at [email protected] or bring it in on the first day of camp.

A YOUTH PROGRAM EMERGENCY MEDICAL INFORMATION AND RELEASE
must be completed, one form per family.
The Youth Program Emergency Medical Information and Release 26-27 form must be filled out every year starting July 1st. The single form is applicable for each ministry listed on the form. It does not need to be duplicated. One form per family.

The Treble Choir rehearses weekly on Tuesday afternoons from 4:15-6:15 and includes rehearsals and musical training in theory and aural skills following the Royal School of Church Music (RSCM) method, "Voice for Life." Registration is open to boys and girls, eight years old and up with unchanged voices.

The Cathedral of Our Lady of Walsingham utilizes the training program of The Royal School of Church Music (RSCM), "Voice for Life." Students progress through different levels as they master musical skills and increase their understanding. Students who have achieved at least the light blue level wear medals with colored ribbons indicating their level of achievement.
The maximum number of form submissions has been reached. This form is currently not available.
